Client Forms Explained

Authorization for Release of information

If you need help with a funding issue and you want us to be able to talk to your financial adviser, bank, or insurance agent, download, print, sign, and send to info@riceandrice.com. You can then have your representative call our office and we will answer his or her questions to appropriately fund or link the asset to your trust.

If you established your Estate Plan with Rice & Rice in the last five years, you probably completed this form at your signing. Feel free to submit the form to the right to verify if we have one on file for you.

Authorization to Contact

Client confidentiality is a very important part of the Attorney-Client relationship. However, there are times when you may want someone to be able to speak with us about your plan or for direction to assist you. With this form, you can pre-authorize us to speak to a specific person(s) in order to help serve you. This does not give them permission to change anything or to by-pass your authority as Grantor and Trustee. There are times, e.g., seeking direction about your Estate Plan’s role in seeking nursing home care, that your pre-authorized person could help you by talking to us.

Physicians’ Statement

Your trust-based Estate Plan includes a provision that requires two physicians declaration that you have reached a point of incapacity. While this is not something we want to think about, you have planned to ease the process to appoint a Disability Trustee to take care of you and your things. When it becomes necessary, the Physicians’ Statement is a key component to allow the transition to take place without involving the court.

IRA Trust/Retirement Plan Trust Information Sheet

Many of our clients are diligent savers and/or people that live within their means. As a result, highly appreciated retirement accounts accumulate faster than they are spent down. This may mean a substantial inheritance for your children or other beneficiaries. The IRA Trust provides a highly protective distribution to maximize the protection, appropriate use, and may limit a big tax hit to your beneficiaries.

To properly complete the IRA Trust, funding is essential. This form helps us to start the funding process.
